DESCRIPTION OF INJURY MEDICAL TERMS:

Right Wrist, Tear involving the volar component of the Scaphoid Lunate Ligament. Fluid distension of the Synovial Sheath of the First Extensor Compartment & some in the Second Extensor Compartment. Diagnosis was Tenosynovitis, First Extensor Compartment & Tenosynovitis, Right Wrist. Small Low Grade Partial Tear of the Volar Component of the Scapula Lunate Ligament. Tenosynovitis of the Extensor Poliosis Breves Tendon at the Level of the Wrist. Partial Tear of the Volar & Mid Fibrous of the Mid Scapular Lunate Ligament. DeQuelvain Tenosynovitis. Longitudinal Split of the Tendon. Right Wrist Sprain & Contusion. Dorsal Subluxation of the Distal Ulna at the Distal Radial Ulna Joint. Tenosynovitis right Wrist & Thumb, Tear of Extensor Tendon & Nerve Damage.

FINAL DIAGNOSIS:

Contusion with severe blunt force to the Radial aspect of her right Forearm, Wrist & Hand.
Chronic Tenosynovitis, First Compartment, Right Wrist with Involvement of the Extensor Pollicus Brevus Tendon.
Complex Regional Pain Syndrome, Right Wrist & Forearm. CRPS
She sustained a Contusion to the Radial side of her Right Hand, Wrist & Forearm. DeQuelvain or Tenosynovitis of the First Compartment with significant Fluid within the Compartment.

Within medical probability, based on evidence-based literature and the sixth edition of the AMA Guides to the Evaluation of Permanent Impairment, her diagnosis is CRPS type 1.

Evaluation of Permanent Impairment edition 6. Therefore, her default impairment of 20% upper extremity is increased to 25% on this reliability as well as the pain inventories- solidifying her ADL loss. The Guidelines in chapter 1 and 2 show that impairment rating is directly proportional to functional challenge or loss. This allows me to increase this impairment rating therefore, within medical probability.

On page 420 of the Guidelines table 15–11, permits conversion of an upper extremity impairment to a whole person impairment. 25% upper extremity impairment is equal to a whole person impairment of 15%.

On page 453 of the 6th edition Guides to the Evaluation of Permanent Impairment table 15–24 and 25 symptoms and objective findings in presentations that are utilized for diagnosis and severity.
